Introduction

Sometimes you can be the last person to know what is really going on in your team or organisation.

This can be due to a variety of factors, including systems, processes, culture, etc that hides information, etc from you about what is really happening.

ii) Are your information sources diverse, ie from all levels of the organisation and from a range of stakeholders?    
iii) Is your support team filtering who you meet and what you read?    
iv) Do you invest time for casual conversations with different stakeholders?    
v) Do you seek opportunities to have informal discussions with different stakeholders?    
vi) Are you naturally curious and ask questions?    
vii) Do you keep an active lookout for background noise that could show a deeper problem and /or emerging trend?    
viii) Do you have a diverse team with different experiences and backgrounds to ensure a range of perspectives are available?    
ix) Do you genuinely welcomed debate and different ideas?    
x) Do you look for learning from mistakes, failures, setbacks, etc?    

NB The ideal answers are 'yes' for all questions except question iii)
