Not Considering What Might Fail

not considering what might fail, ie need to consider what could go wrong and plan for it. It's too late asking what went wrong at the end of implementation. Reasons for potential failure needs to be examined at the start of the project, ie considerable possible reasons system implementation could fail and how that might be handled. This is a continual process, ie thinking about sustainability so that your systems and processes remain relevant and enable better people and business outcomes.
